Five Wishes does not currently meet the legal requirements for an advance directive in Texas as defined in Texas [Health & Safety] Code Ann. § 166.001 to .166. Unlike most other states, Texas requires you use a statutory state form to express your personal wishes. The good news is you can still use Five Wishes in Texas to clearly express how you want to be treated if you are seriously ill and unable to speak for yourself. All you need to do is follow the instructions and complete the statutory form(s) found below and attach it to your completed Five Wishes.
