Five Wishes is an easy-to-use legal document that lets adults of all ages plan how they want to be cared for in case they become seriously ill.  It gives people control over their medical care, as well as the peace of mind that comes from expressing their own wishes and knowing those of their loved ones.

More than eleven million American families are using Five Wishes, and more than 15,000 groups are distributing the document!

The  Five Wishes Video 2007 Edition is ideal for groups that distribute Five Wishes, as well as families that are using the document.  It is easy to follow and clearly explains Five Wishes step by step.  It is already being used by nearly 15,000 organizations and families.

Next Steps

Next Steps, a guide on discussing and coping with serious illness, is a companion to Five Wishes.  The guide includes information on completing Five Wishes, talking to family members about completing a living will, and discussing Five Wishes with your doctor.  It also provides guidance on serving as a health care agent and being at the bedside of someone who is seriously ill.  And it answers commonly asked commonly asked questions.

Five Wishes at Work

Five Wishes at Work is a new program designed for workplaces that are offering Five Wishes as an employee benefit.  These companies receive an Employer Packet that contains all the information needed to promote and distribute the document to their employees and their families.

My Wishes is a simple booklet that helps a child express how they want to be cared for in case they become seriously ill. My Wishes is not a legal document; it is best used as a tool for discussion.

Looking for a way to be part of Hope Today? Even without a pilot project in your community, there are things you can do to reach those in need through your own acts of compassion. We call these small acts of service “Hope Today Connections.”  Each one of us has an opportunity today to connect with someone in need and bring them hope.
