Five Wishes Kentucky

Five Wishes does not currently meet the legal requirements for an advance directive in Kentucky as defined in Kentucky Rev. Stat. §§311.621 to .643. Unlike most other states, Kentucky requires you use a statutory state form to express your personal wishes. The good news is you can still use Five Wishes in Kentucky to clearly express how you want to be treated if you are seriously ill and unable to speak for yourself. All you need to do is follow the instructions and complete the statutory form(s) found below and attach it to your completed Five Wishes.
