Five Wishes does not currently meet the legal requirements for an advance directive in Indiana as defined in Indiana Code Ann. §§30-5-1 to 30-5-10 & §§16-36-4-1 to -15. Unlike most other states, Indiana requires you use a statutory state form to express your personal wishes. The good news is you can still use Five Wishes in Indiana to clearly express how you want to be treated if you are seriously ill and unable to speak for yourself. All you need to do is follow the instructions and complete the statutory form(s) found below and attach it to your completed Five Wishes.
